Map(Bryan) – The Defiance Post of the Ohio State Patrol is currently investigating a three vehicle injury crash on US 127 at the intersection of County Road 15.75 in Pulaski Township.  The crash occurred at approximately 7:45 am on Monday, November 2nd.

The crash involved a 2003 Chevy Venture mini-van being driven by Christopher Smith, age 21, of  West Unity Ohio; a 1999 ADBA cement mixer, driven by Bruce Phillips, age 55, of  Bryan, Ohio; and a 1995 Dodge Caravan driven by Timothy J. Mr. Smith’s vehicle then struck the Parsons’ vehicle which was stopped at the posted stop sign on County Road 15.75 at US 127. Both the Smith and Phillips vehicles went off of the right side of the roadway, where the cement mixer overturned on its side and came to rest in a ditch. The Parsons’ vehicle came to rest on the edge of County Road 15.75.

The occupants of the Smith vehicle included Chelsea Childers, age 21, Matthew Smith, age 1, Madison Smith, age 3, and Melanie Smith, age 6 months.  Childers and Melanie Smith were transferred via life-flight of Toledo. Mr. Phillips was transported to the Bryan Community Hospital and Mr. Parsons was treated at the scene.

Melanie Smith has passed away and Childers is listed in critical condition.

Assistance at the scene was provided by the Williams County Sheriff’s Department, ODOT, Williams County EMS, and Bryan City Fire Department.
